Integrated care in every community

Our health and care needs are changing, with more people living longer often with multiple long term conditions. This new animation explains the challenges facing the health and care system and how partnerships are being formed between the NHS, local government and the third sector in every neighbourhood, town and area in England. These partnerships will make care more joined up, preventative and personal. By integrating services we can look after a person’s whole needs and can help people to start life well, live well and keep well.
