Delivering integrated care: learning from experience

OVERVIEW

Health and care professionals across England are working together to deliver integrated care for their local populations – but it’s not always easy. At this event, you’ll have the opportunity to learn from other teams’ experiences. See where they’ve tried, failed and found solutions that work to deliver joined-up, co-ordinated care for patients.

PROGRAMME

You’ll learn about:

  • how health and care staff are working together across organisations in place-based teams
  • examples of different models of care developing across England that allow providers to work together to deliver integrated care, including integrated care partnerships (ICPs), primary and acute care systems (PACS), multispecialty community providers (MCPs), neighbourhood-based models and others
  • some of the different ways in which integration is being designed and implemented, incorporating integration of acute services, primary care, community services, social care, housing and voluntary and community sector groups
  • the ways in which leadership styles are changing to enable integrated care beyond individual roles and organisations.
