WEBINAR: Co-creation & collaboration with care homes – South West Care Collaborative
Primary care networks (PCNs) across Devon are at different stages of working with care home teams. Different priorities and challenges arise in delivering the Enhanced Health in Care Homes (EHCH) specifications and there is no ‘one size fits all’ solution.
The South West Academic Health Science Network and Devon Training Hub are hosting a one-hour webinar to highlight a variety of approaches for multidisciplinary teams (MDTs) working with care homes.
We will hear care home representatives share their perspectives on collaborating with care homes, continuity of care and skill sharing.
This webinar is primarily for GPs, Nurses, Health and Social Care Teams, Care Home services and welcomes anyone interested or involved in the design and delivery of the new care home specifications set out in PCN Directed Enhanced Services (DES). The webinar aims to support collaborative multidisciplinary learning for the benefit of residents and staff of care homes.
