Our SMART Referral Forms significantly reduce rejected referrals via features including mandatory fields, validation, hide & show, advice & guidance etc.

The DXS Best Pathway clinical decision support solution delivers information relevant to the patients’ condition during the consultation - at the point of decision.

Automated population of forms from the patient record, with pass-back coding, further increases accuracy and efficiencies.

Implementation, including deployment and training, is straightforward and collaborative. Our accessible cloud-based solution means we can do it all for you, we can do it together or you can do it in-house.

The problem

Current clinical pathways are static, complex, and therefore difficult to interpret.

This can often lead to delays in diagnosis, an increase in frequent attendee figures, unnecessary visits to A&E, mental health challenges etc.

It is not feasible for clinicians to consider the patient’s record, symptoms, and overall wellbeing; then make an accurate diagnosis within a typical time-restrained consultation.

Increasingly limited resources within the health and social care landscape result in system burden and stresses that lead to frustration, negative outcomes, and increased costs. The damaging effects of limited system interoperability on Health & Social Care systems adds to an increase in health inequalities and, of course, has a negative effect on carbon emissions.

Evidence

We have a case study showing the benefits of our SMART Referral Forms (see Supporting Documents) based on inappropriate infertility referrals in Ipswich.

Working with Mr. Djavid Alleemudder we helped reduce inappropriate referrals from 37% to less than 2%, and waiting times from 44+ weeks to 22 weeks. .

It is widely accepted there are difficulties in tracking data from primary to secondary care and our SMART Referral Forms are no different. Much of our evidence is anecdotal (via customers using our existing solution) and can be assumed (e.g. use of our clinical decision support reduces inappropriate referral, improves patient record accuracy, reduces decision fatigue, aids levelling-up, reduces carbon emissions etc.).
