Lung Health @home

We are pleased to be working with NHS England to support more people to manage their lung condition at home and to access pulmonary rehabilitation (PR).

This work supports NHS Long Term Plan objectives to increase access to PR and provide rehabilitation interventions to a wider group of people, alongside a focus on supporting systems to recover from the COVID pandemic.

Objectives

Our work is focused on supporting NHS sites to implement and test out approaches to:

  • Improving access to PR through case finding and prioritisation and by improving and personalising the referral pathway
  • Increasing access and participation in supported self-management, education, exercise and other activity for people with a respiratory condition

NHS @home

Lung Health @home is one of several national NHS @home pathways that aim to transform these services. A key part of this is ensuring that people have the right knowledge, skills, confidence, and tools, to play a more active role in managing their health.

Work is being led by the Clinical Policy Unit in partnership with NHS @home and is supported through a number of delivery partners including the Academic Health Science Networks (AHSNs).

Support for systems

NHS test sites will link with their local Academic Health Science Network (AHSN), as well as other delivery partners, to improve how they support people with respiratory conditions to self-manage and to access PR. This will include self-assessment against ‘What Good Looks Like’ for Lung Health @home, identification of a ‘change idea’ and ‘real-world testing’ of interventions and resources. Learning from the test sites will be used to support the development of an online framework to provide practical support and guidance to other areas.
