With elderly and frail patients shielding from external contact, their carers living on site, and limits on care home visitors, video communication has provided a vital link to health professionals.
Our support has included webinars for staff and NHS clinicians, analysis of uptake data to highlight best practice and innovations, and spreading learning rapidly to support care homes to provide effective technology-enabled care.
Webinars – communicating with care homes
In July 2020 we ran a series of webinars with Devon Training Hub to support Primary Care Networks (PCNs) to deliver new care home specifications set out in the Network Contract Directed Enhanced Service (DES).
The webinars cover approaches for multidisciplinary teams (MDTs) to enable effective collaboration and communication with care homes.
We also delivered two webinars with the NHS England and NHS Improvement regional digital team to support the spread of a remote care round approach developed by Kingskerswell & Ipplepen Health Centres in South Devon, and to support care home staff and residents to receive video calls from clinicians.
